A note on tiers and prior auth

  • Tier 1 (preferred generic) is the cheapest. Tier 5 (specialty) is the most expensive — usually 25-33% coinsurance. Same drug can be on different tiers across plans, which dramatically changes cost.
  • Prior auth (PA) requires plan approval before the pharmacy fills the prescription. The #1 reason people skip prescribed medications on MA plans.
  • Step therapy (ST) requires you try a cheaper alternative first.
  • Quantity limit (QL) caps how much of the drug you can fill at once (e.g., 30-day supply only).
